Sentinel by Clearline.

From near-miss to closed corrective action, with nothing lost in between.

Reports go into a spreadsheet. Corrective actions get assigned and quietly die. At audit time nobody can show the loop closed. Sentinel runs the whole loop — report, investigate, correct, verify, record — on one set of data entered once.

The problem
Nobody gets hurt because a hazard was unknown. They get hurt because the report of it was sitting in row 412 of a spreadsheet with no owner and no due date.
01

Reporting is too slow to happen

If filing a near-miss takes a laptop, a form, and fifteen minutes, it doesn't get filed. The events you most need to see are the ones nobody bothers to report.

02

Corrective actions have no teeth

Actions are assigned in a meeting, captured in minutes, and never escalated when they go past due. Six months later the same hazard causes a recordable.

03

Recordkeeping is a separate, duplicated job

Someone re-keys incidents into the OSHA log from a different system, in a different format, months later — and the two never quite reconcile.

What it does

Close the loop, and be able to prove you closed it.

Sentinel is built around the gap most safety programs actually have: not a shortage of good intentions, but no enforced path from the thing somebody saw to the change that made it stop happening.

01

Forty-second field reporting

Photo, location, category, and a sentence — from a phone, with a gloved hand, optionally anonymous. Detail gets added later by the people whose job it is.

02

Investigation workspace

Structured causal analysis with evidence, interviews, timeline, and contributing factors, sized to the severity of the event rather than one heavyweight template for everything.

03

Corrective action tracking

Every action has an owner, a due date, an escalation rule, and a verification-of-effectiveness step that has to be signed before it closes.

04

OSHA recordkeeping

300, 300A, and 301 generated from the same records your team already entered. Privacy cases handled, establishment splits handled, electronic submission ready.

05

Inspections and audit routes

Scheduled walkdowns and self-assessments where a finding becomes a tracked action automatically instead of a note in a binder.

06

Leading-indicator dashboards

Observation rate, action closure velocity, repeat-hazard detection, and heat maps by area, crew, shift, and contractor.

Who it's for

Built for operations where being wrong is expensive.

Clearline Sentinel is sold by the site, not by the industry. These are the operations where it lands first — and the roles that own the problem it solves.

Built to hold up

Designed around what an auditor will ask for.

Compliance is not a module bolted onto Clearline Sentinel. The record structure, the signatures, and the retention exist because someone will eventually ask you to produce them.

  • OSHA 29 CFR 1904 — Recordkeeping and reporting — the 300 log, the 300A summary, the 301 incident report, and electronic submission where it applies.
  • ISO 45001 — Incident investigation, nonconformity and corrective action, performance evaluation and management review evidence.
  • OSHA VPP — The documented evidence a Voluntary Protection Programs application and re-evaluation actually asks for.
  • ANSI/ASSP Z10 — Occupational health and safety management system structure, including the corrective-action hierarchy.
  • Insurance and EMR documentation — Clean, exportable loss and near-miss history for carrier reviews and renewal negotiations.
What it returns

Where the license pays for itself.

These are the three places customers find the money. None of them require a change in headcount, and all three are numbers your finance team can already see.

Recordable prevention

One avoided lost-time injury generally exceeds a year of license cost before anyone counts the indirect multiplier.

Citation exposure

Willful and repeat items run into six figures each, and a missing corrective-action trail is what turns one into the other.

Experience modification rate

A documented, closing safety program is negotiating leverage at renewal, not just a compliance artifact.

Licensing

How to get it before anyone else can.

Clearline Sentinel is not generally available. The way in today is a design partnership: a fixed fee, a product built around your operation, and the first year of license included. The list prices beside it are published so you can budget — not because you can buy them yet.

What a design partnership actually is. You are not buying finished software. You are paying to have it built around your operation, ahead of everyone else, with your name on the decisions. In exchange you get the first year free, a rate locked for three years, and the right to walk with your data if it does not work.

Implementation. Included in the design-partner fee. At list, it would be $12,000–$28,000 one-time depending on areas, forms, and records migrated.

More than one product. Design partners on a second Clearline product pay half the second fee, because the platform underneath is already built for your site. See where each product stands →

Prices are USD, 2026. List figures are planned, not promised, and will be confirmed before general availability.
